Under general supervision, the social worker provides team based counseling and case management services to oncology patients in the setting of private oncology office based practice. Provides biopsychosocial assessment and develops treatment plan based on individual need, facilitates family meetings in conjunction with navigation team, and provides support and guidance to interdisciplinary team for patient care in areas affected by patient/ family psychosocial functioning.
Supports and adheres to US Oncology Compliance Program, to include the Code of Ethics and Business Standards.
Master’s degree in social work from an accredited university. Minimum 3-5 years’ experience as an LMSW or LCSW, oncology experience preferred. Relevant internship experience will be considered in lieu of employment experience. Appropriate state licensure required.
